1. The initial affidavit draft is generated from your uploaded canvassing surveys.

The following 3 steps are critical:

• Be sure there is only one canvasser’s name on the survey. It should be their legal name, not a nickname.
• Check for spelling errors. The name needs to match on the survey and the affidavit.
• The dates entered on the spreadsheet must be formatted MM-DD-YYYY.

2. From the irregularities noted on the survey, an affidavit will be generated with this information and uploaded to your draft affidavit folder.
3. When you see affidavits in your draft folder, confirm that the content is correct. Do draft affidavits agree with what is on the canvassing results spreadsheet or do drafts need revision? Do not revise the house notes on the original walk sheet, however, it may make sense to revise the house notes in the spreadsheet making sure all the associated affidavits reflect the change.
4. Make any revisions on the draft affidavit spreadsheet that are needed.
5. Upload your revision spreadsheets changing only the date in the file name to the current date.
6. Note to data entry person: If you must make major revisions such as a name change or status change (ex. does not live there – to deceased), this must be handled on a case-by-case basis.
7. Notify Toni of the major changes needed by file name and date, and it will be forwarded for special handling.

            Critical:

If there a multiple affidavits per canvassing survey, the notes should be

                 exactly  the same on each affidavit.  If you change or modify notes to try

                 to fit each individual affidavit they will not get processed properly

If you must make major revisions such as a name change or status

                 change such as ( does not live there to deceased). Notify Toni with

                 File name, date and voter ID so she can alert Ro for correct processing.               

3. Upload your revision spreadsheets changing only the date in the file name to the current date.
